Healthcare Associates of Palm Beach is founded by family physicians. We pride ourselves in providing continuing and comprehensive healthcare to the individual and the family. We integrate all the biological, clinical and behavioral sciences. At HAPB, we focus on all aspects of the family; we treat all ages and stages, all genders, each organ system and every disease entity. Everyone is treated with care, compassion and respect.
We provide both urgent and chronic care management, and we are available to our patients 24 hours a day. Same day appointments are available, either in office or via Telemedicine. We have our own certified phlebotomist who does our labs on site, our experienced medical staff always available for help. We look forward to meeting you!

HOW I STAY WELL
As an ex-professional dancer and champion gymnast, Dr Jacobs loves to dance to stay fit. She cherishes spending time with her children, as well as traveling to the University of Florida to watch Gator games.
TRAINING AND PRACTICE
Her undergraduate degrees are from the University of Florida (Associate of Arts and Sciences) and Florida Atlantic University (Bachelor of Science in Nursing). She received her Master of Science In Nursing (MSN)/ Family Nurse Practitioner (FNP) degree from Florida Atlantic University (FAU). She performed a second residency in Pediatrics and earned her Doctor of Nursing Practice (DNP) degree also from FAU. She started her career as a Registered Nurse at Bethesda Memorial Hospital (now referred to as Bethesda East) in 1991. She cared for patients in many different areas of hospital nursing, but spent the majority of her time working in the Emergency Department. Upon completion of her FNP degree in 2002, she joined the practice of Merzer and Sherman Gastroenterology, which merged into Medical Specialists of the Palm Beaches in 2008 (both practices in Atlantis, Florida). In 2013 she had the unique opportunity to move to Northwest Georgia to help build a family-centered medical home at the Shaw Family Health Center. Dr Jacobs has been a Primary Care Provider (PCP) at Shaw Family Health Center in Dalton, GA from 2013-2021. She is very excited to come back home to Boynton Beach, FL and have the opportunity to continue providing exceptional evidenced-based multi-cultural care to patients at Healthcare Associates of Palm Beach.
